PASSPORT PROGRAM

Helping Adults Access Their Communities

The Passport Program, funded by the Ministry of Children, Community and Social Services (MCCSS), helps adults with developmental disabilities be involved in their communities and live as independently as possible by providing funding for community participation services and supports, activities of daily living, and person-directed planning. The program also provides funding for caregiver respite services and supports for primary caregivers of an adult with a developmental disability.

CLH DSS is a Passport Transfer Payment Agency (TPA) for those enrolled in the following Programs: Supported Living Homes (SLH), Host Family Provider (HFP), and Supported Independent Living (SIL). If you are a part of one of the listed programs then you are eligible for CLH to oversee your Passport funding, should you choose CLH as your TPA.
