Complex Support Clinical Coordinator

The Complex Support Clinical Coordinator (CSCC), under the direction of the Community Networks of Specialized Care – Central East (CNSC-CE) Manager, will be responsible for providing clinical support services for people as part of the capacity building within and between systems. The position will provide cross-sector coordination and support including facilitating the acquisition of required support and services, coordinating support to enhance existing resources and services, liaising with different sector partners, including those with the criminal justice system, health system etc. and assisting with system navigation, including urgent response and other planning tables. This position services Haliburton, Kawartha, Pine Ridge and Durham.

Duties and responsibilities will include, but not be limited to:

  • The CSCC will make referrals and facilitate access to and from specialized resources, including at the local, regional and provincial levels. This would include clinical support, psychiatric support, psychological support, and specialized accommodation.
  • Providing evidence-based person-centered coordination of support including development of plans and linkages to services for persons with complex multisystem needs who require a high level of active engagement and ongoing monitoring.
  • Provide enhanced coordination and develop clear support plans in partnership with other agencies/service providers that address urgent and complex needs of identified people.
  • Assist the person, caregiver, and service provider to develop goals and objectives, obtain the required resources, services and support to help identify alternative resources to meet service gaps.
  • Facilitate planning, and coordination of services within and across sectors including clinical services and other services as appropriate.
  • Clinical training facilitation for service agencies as well as program development and implementation for clients and service agencies.
  • Liaise with community agencies, service providers and community partners as well as other professionals.
  • Assist in planning and discharges for ALC clients in hospital settings.
  • Provide consultation and collaboration with community partners who are outside the Developmental Services sector.
  • Identify specialized training needs and recommendations for community support.
  • Cross-sector clinical coordination support (e.g. assessment, clinical planning, housing, planning tables).
  • Facilitate the acquisition of required supports and services (e.g., specialized services, residential, etc.).
  • The CSCC will ensure adherence to the CNSC mandate.

To be considered as a candidate, the successful applicant must have the following minimum qualifications and experience:

  • Completion of a formal university degree in Social Services, (BSW) at the undergraduate or postgraduate levels, preferred.
  • Minimum 5 years’ clinical experience in developmental disability and/or adult mental health services.
  • The preferred candidate maintains a professional membership in good standing with a regulated college.
  • Working knowledge of logic model, bio psychosocial model.
  • Demonstrated complex support clinical coordination abilities and a familiarity with how to access support and services available to adults with developmental disabilities and high support complex care needs.
  • Demonstrated ability to work as part of an inter-professional team and ability to work in a cross-sector context.
  • Demonstrated ability in adherence to people’s rights and an understanding of people’s ability to direct care.
  • A valid province of Ontario’s driver’s license and access to a reliable and insured automobile for transportation to be used in the course of employment as travel across Central East is required.
